If you would like to pay on-line (via Visa or MasterCard), please complete the on-line registration form below. After completing the form and clicking Submit, you will be directed to our American Cancer Society donation page for secure payment processing.

If you would like to pay via check, please print and mail the attached form along with your check. Please do not complete the on-line form if you are not paying on-line.

As a participant in Relay for Dogs, I, for myself, my executor, administrators, and assigns, do hereby release and discharge the American Cancer Society, Relay for Dogs, the event site, their management, their officers, members, sponsors, organizers, or their representatives, or their successors, and all cooperating businesses and organizations from all claims of damages, demands, actions, and causes whatsoever, in any manner arising or growing out of my participation or that of my dog/dogs in this event.

I give my full permission for the use of my name and photographs associated with this event.

I understand that I must provide written documentation from my veterinarian that shows proof of my dog's up-to-date rabies vaccination and city permit (if I live in the city of Columbus). This must accompany my dog for admittance to the event.

I will bring only leashed dogs to this event and I understand that I am responsible for my dog's behavior. Agressive and unruly dogs will be asked to leave. Dogs must remain leashed at all times. I also understand that I am responsible for cleaning up after my dog.
